Request for Service
If you would like to request service that allows DaySmart Spa to access your business and/or personal computer and/or customer information, you must fill out this form granting permission to access your data.
Print Approval Form
When you have reviewed all digital and hard copy proofs of your gift cards and are ready to approve the cards to be printed, you can fill out the Print Approval Form and printing will begin on your cards immediately.
Return Hardware
Return on hardware (cash drawers, printers, bar code scanners, etc.) if the hardware was damaged in shipping or is defective upon initially receiving the item(s). Your situation will be reviewed by DaySmart Spa.
Serial Number Recovery
If you have misplaced your software serial number and need it reissued, fill out the Serial Number Replacement form to verify software ownership and your serial number will be reissued to you.
Database Password Recovery
If you have forgotten a database or employee password and cannot proceed without entering one into the software, we can attempt to recover the password for you.
Update Account Information
If you would like to update or add information to your DaySmart Spa account you may fill out the Update Account Information form to verify ownership of the software.
Account Password
If you would like to add, modify or remove a password to your DaySmart Spa account to limit accessibility to technical support, billing services, updating information, and information to any other DaySmart Spa Service.
Domain Name Transfer
Complete the Transfer of Domain ownership in order to transfer a domain that is currently registered to DaySmart Spa into your own name. This will enable you to host your domain on a separate server with a different web hosting service.
Transfer of Ownership
If software by DaySmart Spa is sold or given to a new owner, the ownership of the software must be officially transferred by completing the Transfer of Ownership form.
Data Retrieval Request
Complete this form to request temporary access to data that either remain on DaySmart Software Inc.’s servers or to request a temporary license to open your installed software.
Transferring Data
This form is to request and approve the transfer of data from a listed account owner’s database to another account’s database.
Rebate Request
Complete this form to request a rebate for the purchase of a VP3300 mobile card reading device. To be eligible, you must have processed a minimum of $1,000 in credit card transactions through DaySmart Spa.
